A 50% deposit is required to secure participation and begin processing your registration for this program.

You will receive a participants questionnaire via email.    

After completing and returning  the questionnaire, you will receive a follow up phone call to: 

  • Discuss your questionnaire and make sure all pertinent information about your practice and optical has been obtained.
     
  • Give you instructions for your DME workbook.  You will have to download some materials from CMS and your Region’s DME MAC.  You will be given full instruction on how to do this.  The workbook, instructions and certain contents will be mailed to you.  The workbook must be completely assembled before the day of the Seminar.
     
  • Book a day and time for the Go-To-Meeting Seminar. 
     
  • Register personnel from your office who will attend the meeting/seminar.
     
  • Review materials to be used at the meeting/seminar.
     
  • Instructions for Seminar Day.
